General Surgeon 's  Response
Hello,

* As per my surgical experience, MRI report with narration of L 4 L 5 dessication, reduced disc height and postero central and right para central disc hernination indicates changes secondary to age related osteoarthritis and sequences.

* As there are no changes in signal intensity and other issues of wedging with abnormalities, chances of any grave pathology such as metastasis or others are not there.

My guidelines at present

- control of body weight and cholesterol levels as obesity will worsen the above mentioned effects
- heating pad application over the lower back portion
- avoid sudden bending or weight lifting
- local application of analgesic spray or ointment with diclofenac or capsaicin content
- anti inflammatory and analgesic medications as etoricoxib or others if not contraindicated and hypersensitivity issues
- postural adaptations
- assisted physiotherapy with experts.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel
