Brief Answer: You should see a neurosurgeon.

Detailed Answer:
Hello,
Thank you for using HCM.
Small marginal osteophytes and vertebral bodies end-plates changes indicate lumbar spine physiological attempts to provide stability in an unstable spine due to inflammatory degenerative process. L4 - L5 disc herniation represents a specific situation of intervertebral disc damage due to trauma or chronic degenerative changes, resulting usually in spinal canal narrowing and spinal nerve compression, perceived as lumbar pain.
You should see a neurosurgeon for a better assessment and further follow-up or possible surgical correction.
Meanwhile, you can take anti-inflammatory pain-killers to manage your pain, such as Ibuprofen 400 mg three times/day, after meal, or Paracetamol + Codeine (500 mg/ 30 mg), at maximum four times/day.

Hope this is helpful.
