Large head sperms with prolonged liquefaction time of semen indicates underlying significant pathology which may be enough for producing infertility issues in majority of the cases. The structural protein formation in the constituents of the semen are vital for this problem and one may has to think of testicular dysfunction from various reasons. In my clinic, after thorough clinical examination, I do recommend a color Doppler of the testicles followed by testicular biopsy as per the indication.
