What Does This Ultrasound Scan During Last Trimester Indicate?

Hi mam... i took scan by 31 to 32 weeks it showed left lateral placenta extending lower segment but not covering os. Grade 3 maturity. Fetal weight is 1800 gms. AFI is 12. Second baby.. first is c sec. Is there any possibility of normal delivery. Cephalic presentation. . Feels pressure in pelvic bone.

Hello,

Grade 3 placental maturity generally happens after 37 weeks of pregnancy and in your case because you have a grade 3 placental maturity at 31-32 weeks of pregnancy, this is an indication that you and your pregnancy will need to be closely followed by your doctor due to early aging of the placenta and possible risks on the pregnancy. There is no agreement within doctors what specifically aging of placenta means clinically but it is sure that this is an index of higher risk pregnancy and close monitoring is needed.
As for the AFI and fetal weight it seems to be normal. Generally after a first C-section it is recommended to continue with C-section in the second pregnancy due to risks of uterine rupture being higher. Some women do dare and choose vaginal pregnancy in the second delivery after first C-section and most do fine but the number of women that have uterine rupture with fetal death and risk for their own lives is higher.

Having a low placenta also might prevent you from having a vaginal birth especially if the margin of the placenta is not more then 2.5 cm away from the cervical os.
