What Does This X-ray Report Indicate?

Have CHF and COPD. Went to my pulmonary doctor yesterday. No fluid retention; lung x-ray shows no change in lungs since last year; taking diuretics and using inhalers but last several days have been a real struggle to breath. Could something else be going on?
