What Is Causing Scortum Pain?

I have recently been having several issues; my scrtoum aches severely during and after orgasm, my orgasm however is not accompanied by ejaculation as my ejaculate leaks out sporadically prior to orgasm, and I have been able to achieve a rigid erection but often can not maintain it especially during vigorous sex where I get slihtly winded. I am 50 yrs old and had a vasectomy at 40 which also significantly changed my ejaculations volume.

Hi,Dear,Thanks for your query to HCM.
Dear I read facts of your query and reviewed it in context to your query health issues.
I understood your health concerns and feel Concerned about them.

Based on the facts of your query the reply of your query is-
Your Scrotal pain is from-retained sperm ejaculate post-orgasm, as the blocked Vas deference from Vasectomy, would increase the retrograde pressure from retained and secreted sperms from the normal testicles in the scrotum.
Or it could be from PVPS-Post-Vasectomy-Pain-Syndrome.

Other causes need to be ruled out like undiagnosed sperm granuloma/ vasectomy stitch granuloma and resolving post-vasectomy testicle hyperfunctioning. So you have to be careful and need to attend it seriously.

Do's -suggested for your case-
Don't Selfmedicate, as is done now by you- it could complicate your health problem more.
Don't worry for the reduced ejaculate volume- as it would be less after reduction of sperm contents-which is obstructed after Vasectomy.
Do Consult Uorologist / or Surgeon-besides these suggestions,as things depend on your submissions, which could be different than what you observed.
Tab Vit B12 / Folic acid to improve fast the recovery.

Blood Tests / USG Study of Scrotum and review with Surgeon / or Urologist would be needed if no recovery with initial treatment given, to rule out other causes of it.

Just don't worry and be patient and co-operate with your doctors, till you verify with your attending doctors.
Need to Check with your Surgeon / or Urologist- if things don't recover.

-Other causes need to be verified with your family GP doctor and team of Specialist doctors, as the facts may be different what you submitted.

-For this Second opinion from PCP-or Gp doctor and if need be with Surgeon and Urologist-specialist-is needed who would do proper investigation after proper physical assessment to resolve your health issues.
-for early recovery-please follow above do's which would resolve your complex health issues.

Hope this would help you to treat your health issues in the best way possible.

Welcome for any further query in this regard.
Good Day!!
