Hi, Eric,
From history it seems that your friend might be having acute infection in his middle or external ear might have produced referred pain in the neck or there might be having enlarged tender neck lymph nodes due to ear infection giving rise this pain.
He might require one course of antibiotic medicine with prescription ofyour doctor.
Meanwhile give him ibuprofen for pain as and when required.
