General Surgeon 's  Response
Hello!

Thank you for the query.

Fingers are long enough to reach some part of rectum only. This part is located outside the abdominal cavity (extraperitoneal location). So even if she has causes some wound in the rectum, this wont give typical perforation symptoms. It would rather give an abscess in the rectum area.
Small perforation can be caused by diverticulitis, regional ischemia of the intestine or due to bowels obstruction which last for quite long time.

Hope this will help.
Regards.
