Cardiomegaly means the heart is enlarged, but there are many causes of an enlarged heart with varying levels of significance. A heart can be weak and ballooned out and pumping very poorly from high blood pressure or chemotherapy or viral illness, this is also called non-ischemic dilated cardiomyopathy. Or the left ventricle can be enlarged and thick from high blood pressure, which can be called LVH or left ventricular hypertrophy. Medicines can treat all of the above, sometimes it improves, sometimes it doesn't. It really varies widely depending on the cause of the condition. Hope this was somewhat helpful.
