what is the next line of treatment for necrotizing pneumonia when vancomyacin is not working, based on cxr that shows no change or possible worsening after 4 days of vanc and other a/t therapies? the pt is 31 y/o female who recently had type a flu. no underlying conditions. she continues w/low grade fever, wbc-11. no resp. distress/sob. no hemoptysis.
HI Thank for asking to HCM This what the x-ray shows but what is about the clinical condition? x-ray finding will not give any clue about the clinical condition neither this will show the regeneration of parenchymal cell, more over if you calling it necrotizing tissue then the tissue which are already necrotized will show the image of some opacity even after the total regression that does not means that the antibiotic failed to respond, the important is clinical condition if this is being improving then this shows that the drug is working properly, on the basis of x-ray finding it is not ideal to change the drug, I strongly hope that you will be agree with this, I would be glad to answer the next query have good time.
