It is possible may be due to rubbing hard at the part,
But most probably he might have some infection in anal region or fissure in ano.
Go for antibiotic course and anti inflammatory medicines.
He will be alright.if not cured,consult surgeon.
Avoid constipation.
TRake more green veggies and plenty of water.
