Understand your concern
PCOS is very complex disease. Main reason is
insulin resistance, hormonal imbalance that lead to irregular period, no ovulation/
ovarian dysfunction.
Yes PCOS cause
infertility problem as period and ovulation is irregular.
I will mange PCOS case as below:
First get the done following investigations: reproductive hormone analysis, Blood sugar,
Lipid profile and
ovarian follicle.
Now following are the aim of treatment:
Regularize the period, reduction of weight and increased insulin sensitivity, induction of ovulation, cure the hormonal imbalance and treatment of hirsutism.
Oral
contraceptive or primolut N: should be given for at least 6 months to regularise the period
Metformin: It maintain glucose homeostasis and also induce the ovulation
Spironolactone: Also help in the induction of ovualtion
Hirsutism should be treated by topical eflornithine
If not respond to above drugs following drugs can be stsrted"
Clomiphene citrate: aloso induce the ovulation and help in maintaining fertility
Ovrain surgery like dreeling is the last option if medicin will not work.
If fertility occur in the future: FSH analogue IVF should be used
Diet and life style modification:
Drink plenty of waters, eat helthy diet, fresh fruits and green leafy vegetables, avoid excessive sweets and yogurts, Avoid fatty/spicy/salty foods
Reduce weight by 10-12 percent by regular exercise like walking. running, swimming, cycling and yoga.
