I'm sorry, but I'm afraid that a patient in her condition would come out of coma, or even if she does, long enough to be significant. There are certain things that help decide management of patients with hepatic cancer. firstly, the size of the tumor, and the no. of tumors in the liver, secondly the child class, and thirdly the functional status of the patient. For a patient who's tumor has metastasized, the treatment options are already limited.
As of the discharge planning, aim for treating the
hepatitis B,esp in patients in whom treatment of the cancer is being considered, prevent further episodes of
encephalopathy with
lactulose or other agents, address the presence of
ascites with diuretics if possible. advise adequate caloric intake in the range of 2200-2400 calories, with a limit on proteins from animal sources.Advice adequate activity, possibly physical
rehabilitation to improve the functional status.
