What Leads To Unconsciousness,breathlessness Despite Suffering From COPD?

hello, I am worried about my father, he is prescribed much mediaction and had been diagnosed with COPD; I have begged all the hospitals and DR s why he keeps passing out unconscious hard to breath and he takes a fentanly patch , i just looked this up with many other medications, blood pressure , sleep medication, other pain meds... the worst I see is the side effects of fentanyl patches, I am POA of my fathers medical; yet I can not get one of his DR help and he has been hospitalized three times this month and we find him on the floor last major times it was htree days missing had to send a welfare check he was in his apt on the ground very low hert rate.... what can I do
