Long history of migraine; palpitation and higher diastolic ( around 100). Has been on TENORIC 50 for many years; but of late palpitation has surfaced with a feeling of fatigue. Acute head reeling. what medicine would you suggest? Patient is 70 years old. Female

Take the patient to an Internist / get examined / basic investigation may be necessary... to investigate the contributing cause – like anaemia, postural hypotension, ear problem, Brain etc…
Stepping up the dose, addition of a third drug are some of the options
