What Should Be Done If There Is Testicular Torsion And Twisted Spermatic Cord?

So I think I have testicular torsion. They say it's an emergency and all but I feel as though I've had it as long as I can remember. On my left side it feels as though my spermatic cord is very twisted. And again it has been this way for quite some time. I also did the cremasteric reflex test (though rather unprofessionally) and it seemed to work on the unaffected side but not the left. I have never felt any out of the ordinary pain though the testicle does lay almost vertical at times. Any advice?
