Hi Dear !! Thanks for your query to HCM .
Read and reviewed your query and health concerns.
My opinion-
IN the given situation, Your dad seems to be having-Multiple ailments related to Obesity-like-2 ligations for varicose @ age of 36/ 45 yrs/ with Pulmonary hypertension-which seems to be accompanied pulmonary embolism.
Along with this he has recent 4 stents for the PAD?peripheral arterial disease, from High lipids mostly.
Drop in the blood could be due to accompanied multiple medicines, which cause orthostatic hypotension(especially high bp medicines).
Thus the high bp in your dads case is from accompanied obesity with high cholesterol and lipids, leading to arterial leg disease with high bp and also causing-Leg DVTS with accompanied pulmonary embolism which lead to Pulmonary Hypertension in your dad.

This briefing reply is on facts submitted, which need to be verified from your attending doctors, with whom you can check before starting further changes in the treatment of your dad.
Hope this would help you to plan further of this complex illness of yours.
