What Type Of Hysterectomy Is Advisable For Glandular Dysplasia In Uterus?

I am a 54 year old who has not gone through menapause yet. 6 months ago I had my first abnormal pap and biopsy which showed glandular dysplasia in uterus. Repeat 3 months showed nothing but last week it was back. Have had an ablation about 4 years ago but continued with monthly cycle. Because of my age my Dr. Is recommending a total (tubes and ovaries included) hysterectomy. What type of hysterectomy is best? I am worried about leaving anything behind but Dr. Said we can do this lapriscopically. Any advice would be greatly appreciated. Dr. said this was preventative but with ablation in past we just don t know. Also transvaginal ultrasound looked ok.

Hello,

Glandular dysplasia is a precancerous condition. The following are treatment options:
1) You may only need careful follow-up by your provider with repeat Pap tests every 6 to 12 months.
2) If the changes do not go away or get worse, treatment is needed.
3) Cryosurgery to freeze abnormal cells
4) Laser therapy, which uses light to burn away abnormal tissue
5) LEEP (loop electrosurgical excision procedure), which uses electricity to remove abnormal tissue
6) Surgery to remove the abnormal tissue (cone biopsy)
7) Hysterectomy (in rare cases)

Endometrial ablation has nothing to do with this. You can discuss the above modes of treatment with your gynecologist and then decide about surgery. You don't need to worry about hysterectomy as it can be done laparoscopically. Ovaries and tubes can be retained as glandular dysplasia is only confined to uterus.
