Hi dear user
Swollen tonsils is a signs of an infammation but white spots in tonsils in my opinion is a sign of a bacterial infection in your tonsils and this means that probably the antiinflamatory non steroidal drug(ibuprofen) is not sufficient and it is right that at this point the doctor will give you antibiotic tablets or injection.
Hope I have answered your question. If you have any further questions I will be happy to help.
Wish you good health.
