Why Is A Surgery Not Performed For A Severely Blocked Carotid Artery?

My husband has been very hot in the head last night ,nauseous, has been in the Royal Brisbane Hospital from the Bundaberg Base Hospital he had same problems only had shaking all over when taken in. They said he had more than 90% blockage of the right carotid artery ,and tried him on drug therapy.Why won,t they operate. does he have to have a massive stroke before they will do anything!
