Why Is Hair Drug Test Stating Positive On All - 30, 60, And 90 - Sections?

I took drug within past 90 days, but not within past 30 or 60 days, so why is hair drug test stating positive on all - 30, 60, and 90 - sections? Results are below cutoff level for 30 and 60 days, less than 100pg/mg, but still listed as positive on lab report.

The timing on the test is inexact. They are splitting hairs.
Several known possible reasons it can misfire:
1) some drugs stay in the system and get deposited into the hair over weeks/months even after quitting (heavy use of marijuana).
2) The hair can come in direct contact with the drug (powdered cocaine, heroin, methamphetamine... anything snorted) and then external drug lights it all up.
