Why Oxygen Is Not Reaching Till Lungs Without Ventilator?

my father-in-law aged around 60 is a kidney patient (for which heamo-dialysis is on-going since 2 years) and has currently gone through by-pass surgery on 13th. since then he is on ventilator. as soon as the doctors are removing ventilator after 3-4 hrs CO2 starts getting accumulated. it seems oxygen while taking without ventilator is not reaching till lungs... kindly suggest how to proceed or who is the best doctor to consult

Patients with kidney failure undergoing cardiac surgery is always an added risk for the patient.Because of the kidney failure, there is always chance of accumulation of water in lung tissue which prevents oxygenation and ventilation preventing carbondioxide to come out from blood leading to accumulation of CO2 and low oxygenation.
The patient will require to undergo dialysis if required.
Any standard cardiac surgical center can handle these patients appropriately with consultation with nephrologist
