Can HIV Be Contracted Through A Paper Cut On The Finger During Protected Intercourse?

Question: Hello Doctor,
I had sex with Sex Worker (Safe sex only-used Condom).

Before sex I was caressing her private part(Don't remember which finger I used) and I did not insert my finger in her private part.As far as I remember there was no fluid from her private part.

1.The problem is I had PEELED/BROKEN skin due to cold with paper cut in my middle finger. Is there any possibility for HIV transmission if my finger with Paper cut touched her fluid unknowingly? Please confirm this point

After coming back home I washed my hands,at that time in my middle finger there was small bleeding (But only after washing,not before that and amount of Blood was not even equal to one Single drop).

Please help me and I am confused totally due to this.

If you truly had an open wound and it came in contact with her body fluids, and she was HIV positive, then at least in theory, it is possible to have contracted the HIV virus. However, based on your description, it seems quite unlikely that you would have caught HIV or any STDs from her.

Just to reassure yourself, I would suggest that you get tested, but it seems unlikely to me that you would have gotten HIV.

Hello Doctor,
Thanks for the reply.It was not an Wound. It was peeled Skin with tiny Paper cut.

Can Hiv Virus enter via Peeled Skin/Paper cut ? There was no noticeable fluid from her body.
I suspect that I might have touched the Minimal fluid (I may not have felt due to minimum amount).

Also is it possible for a sex worker to have sex without cuming. The reason is I could not see the sex worker ejected fluids in noticeable amount.

I am living in Japan now and planning to do test after 4 and 12 weeks of exposure. However please let me know the test I can do ASAP in Minimum period (2 weeks to 3 weeks) .That will ease my mind.I am unable to concentrate on regular activities and not able to take food.Please do advise.

Hi,

It is possible for her to have sex without having an orgasm and for her body to secrete vaginal fluids as well. If you were bleeding from your wound then at least in theory, it is possible to have contracted the virus from her.

However, it is very rare to get HIV without some amount of penetration and it sounds like that did not happen. It is also good that there wasn't any real exchange of bodily fluids. However, I can't give you total 100% reassurance that it is impossible to have contracted HIV. That is because the wound you describe, the paper cut, if it had even a small amount of blood means that it was open, and there may have been some bodily fluid from her that touched it. This means that there is a possibility to have contracted the virus, even though I do believe that it is extremely unlikely.

The testing for HIV can be done right away, but it is not really useful until four weeks after the exposure. Most tests will be positive by then if you did get the virus. You should wait for 4 weeks and then do another confirmation at around 3 months.

Thank you very much for the detailed response.

1.The bleeding happened only after I washed hands and pressed the Finger to check how deep the cut is.Only after I pressed finger area,very small amount of blood appeared along the cut.

2."it is possible for her to have sex without having an orgasm, and for her body to secrete vaginal fluids as well" - Here you mean it is possible for sex worker to complete the sex without having Vaginal Fluid ? I understood the first statement "w/o having an orgasm"

3.In another thread it was mentioned that "HIV RNA PCR test or DNA PCR test" can be done within 7 days (just for initial test) and to confirm this result do another test 4 weeks after exposure.Can I take above test after 10 days? What is your opinion doctor.

4.Have you ever heard person contracting HIV via Finger cut or Peeled/Broken skin ?

Hi,

Based on your description of the paper cut, I really don't think you have anything to worry about. As far as vaginal fluids is concerned, there are always natural vaginal secretions even if a woman is not having sex at all. The test can be taken after 10 days and might show a positive results if indeed it was positive, but a negative test could just mean that you haven't yet made enough antibodies for the test to show it.

By 4 weeks, 95% of the time, if you were positive, it would show. By three months, that number is close to 100%.

You can take the test as many times as you want, but I personally would just do it at 4 weeks and at three months. Otherwise, you are just making yourself nervous for nothing. It sounds to me like you will be fine, but taking the test still makes sense.

Hello Doctor,
I have decided to do PCR RNA/DNA HIV test and at the same time I understood that test after 3-4 weeks and 12 weeks is necessary to come to conclusion.

Exposure happened on 29th Dec.In Japan they are telling to do this test 11 days after exposure.

I also want to do asap.Please confirm whether doing test on 6th XXXXXXX makes sense or not.

I have two more dates 7th and 11th XXXXXXX Please let me know which date I can do PCR RNA/DNA test among these three dates (6,7,11)

I can only imagine your anxiety. I do believe that of the three dates you gave me, it makes the most sense to take the test on the 11th. The other dates were less than 11 days from the exposure and won't be as reliable.

If you repeat it in 12 weeks you should be fully reassured. Based on your description of what happened, I don't believe that the risk of transmission in your case is very high at all, but it still makes sense to get the reassurance of the testing.
