Question: Hi there,
I had the following situation.
1. Had anal sex with a prostitute using condom
2. Then did fingering with middle finger (put midle finger in her vigina)
3. Then washed hands with normal water (no soap/sanitizer etc, only water)
4. Then washed penis with the same hand (no soap/sanitizer etc, only water)

Do you see any risk of HIV exposour? and what will you suggest next? All this happend on XXXXXXX 23, 2016

Thanks for posting your query.
You had protected anal intercourse. So your risk is almost nil through this intercourse. Fingering caries only a negligible risk as per CDC XXXXXXX Washing your penis with the same (washed) fingers also carries almost nil risk.
So you need not worry much.
But anyway, as you got disturbed in mind, it is better to have HIV test after the window period (12 weeks), get ruled out the least possibility of HIV before you resume unprotected sex with your regular sex partner.

I was planning to do the following:

1. Do HIV Early Detection test from STDCheck.com (on the 10th day)
2. Do HIV test after one month
3. DO HIV test after 3 months
4. Do HIV test after 6 months (for HIV-2)

I would like to hear your opinion on the above and also your opinion about STDCheck.com (if its trusted/certified enough to do the HIV Early Detection test).

Welcome back
Your risk for HIV /STD is negligible. STD check. Com is a private concern and profitable body. Their recommendation is not an absolute one. My opinion from the 30 years of experience, I recommend the following :
HIV early detection test by 10th day would not be very much conclusive and can be skipped. HIV 4th generation COMBO test(detects both antigen and antibodies against HIV 1 and 2 infection) 3 months after exposure will be sufficient to rule out HIV infection. It will be highly reliable and conclusive. Test after 6 months is not necessary. Tests for STDs can also be done at the same time (3 months). You will be likely to have all negative results. I will follow your recommendation, however i might consider early detection test based on my fatigue level in next couple of days (which has already became low after i saw your comments). Today however, i sightly had dhiria/motion like situation (but that could be because of food) and also had flu+fever live situation for 4-5 minutes (which takes my fatigue high, but it might not be related).

The early detection test could bring me more relief if it comes negative hopefully (although it will not be conclusive), but i will make that decision later in few days (Please let me know if there is downside of Early Detection test other than money waste).


Following further details on how i got caught/trapped into this situation:

2 months ago, all of a sudden i got ED problem which i never had before. I did wait for 3-4 weeks and focused on diet/excersize etc, but that did not helped. Then i wen to my physition/doctor and she suggested some harmoon/blood tests which came clean except for Vitamen D defeciency, but Vitamen D pills did not helped. Then she suggested some Ultra sound and X-ry which i did, however the results are still awaited.

In the mean time i was doing research for ED on internet, and somehow i came to the conclusion that it/ED could be a physiological issue and took this extreme step (going for an other women which i never did before), and now i got caught into this new situation.

The ED issue is still not resolved and also it does not seems to be physiological and now i am waiting for the X-ray and Ultrasound test results and my doctor advise.

I can understand your concern.

HIV Antigen detecting tests (P24 antigen test, PCR tests) can detect the presence of HIV as early as 1 week after the infection, if any, even though we can not take it as a conclusive one and it can be taken as conclusive by 4 weeks as far as HIV 1 infection is concerned. So you can take up this early detection test and it can definitely reduce your anxiety.
Your symptoms are unlikely be due to HIV as the earliest manifestation in HIV can occur only after 3-4 weeks after the infection and not earlier.
Regarding you ED problem, nobody can get erection just by a thought alone as our other motor functions, it will have to occur spontaneously on exitement or stimulation. You are at the age of 42, and you can not expect a just a thought of a girl can stimulate you. Only a hug or kiss or intimacy can stimulate you. Moreover if you are concentrating on your organ you can not get the erection properly, but you can get good erection when you focus your full concentration on the pleasure of touching a girl's body and the act of sex.
When you have early morning erection or nocturnal erection or emission then you are sexually normal. If too much worry or exhausion, that also will decline your erection. Consult a good sexologist and get well soon.

Thank you so much Dr. S. XXXXXXX for the additional information/advise on the ED issue and i will follow it as well.

I am planning to go for the HIV Early Detection Test (STDcheck.com) on Feb 03 10am (10 days and 16 hours after the exposour) and see what the result is.

I got the following information from STDCheck.com:

"Our HIV RNA early detection test (HIV RNA Qualitative by TMA) detects the HIV virus directly, reducing the time between infection and detection to 9-11 days. The test will be approximately 95%-99% accurate at this time frame. It is the most accurate and the most sensitive test on the market. It is also the only early detection test that is FDA-approved."

Please let me know if you have any further suggestion or comments. Else i will close this thread with many thanks for all your kind suggestions and help. Thank you.

Of course HIV RNA Qualitative PCR is a very good sensitive and reliable test. No doubt. It can detect the infection as early as 1 week itself. But in very rare instance where the infection gets established a bit delayed due to various reasons like local tissue immunity etc., it could be very much conclusive by 3 weeks.
