Brief Answer:
TMJ disorder, Ear disorder, Consult oral physician

Detailed Answer:
Thanks for your query. I have gone through your query.

As far as the lightheadedness and plugged ears is concerned, it more likely would be related to ear problem like wax in the ear than temporomandibular joint disorder. The common symptoms of temporomandibular joint disorder(TMJ), are limited mouth opening, clicking sound on opening and closing. I hardly ever saw a patient with TMJ disorder presenting with lightheadedness and plugged ear. Therefore I will suggest you first visit an ENT surgeon to evaluate for cause of plugged ear and lightheadedness.

You can get TMJ problems sorted through an oral physician. Meanwhile you can use hot fomentation over the joint as well as use over the counter pain medicine and continue to use night guard.

I hope I have answered your query. If there are any more queries, revert to me.

Take care.
