It does not look like herniated again. However there is a kind of skin flap on the left side which can be a small hernia. After a surgery it is much harder to tell especially whithout a palpation.
Do you feel any hardness or pain in the mentioned area? If so, has it appeared recently?

the skin flap meaning the peace thats moving in and out or the piece at the bottom? the piece on the bottom left is just a hemmatomma still.

Yes, the piece that is moving in and out. Either the skin is not attached to the abdominal wall or its a tiny hernia. If its a hernia, you should be able to feel a hard lump in this area. If its soft, hernia is rather not the case.
But like I have said, its hard to say not being able to palpate it. Generally, wait and see is a good option. 3 months after the surgery is the earliest term for another surgery. And its the best time to see if there is a hernia or not. Now its hard.
If it bother you, ultrasound can tell.
