Posted on Mon, 12 May 2014
Question: Sir, I on 27th OCt-2013, done unprotected oral sex and had cunniling her vaginal part(around) through our tongue for maximum two/three minute. She also sucked my penis (covered with condome). I also took bath with her with warm water. I had not vaginal/anal sex or full intercourse. In that time I did not felt any things on my tongue. But after some time (around 30 min), I felt some sticky lubricant on my tongue. Immediately I washed my tongue with warm water. exposure- 27-10-13 1. First feeling- tasteless/dry mouth/tingling/inflamtion/pain in mouth and tounge – After 4week/29days (on 25-11-13) 2. Night sweat started – After 4week 2. Felt vague feeling/wiron in body- 4th week 3. Mild fever(98degree) for 3-4 days- 4th week 4. Cold blister appered on corner of left lip- 6th week 5. Swollen Lymph nodes(under the both jaw line) and pain – After 7th week/ 52days 6. Having muscle ache/pain in both shoulder & thigh and weakness. It is almost 25week(175 days) passed but burning/tingling/pain in mouth, swollen lymph nodes under the jaw line, night sweating and some irregular pain in shoulder and thigh muscle are being continued Still I have not gone for HIV test. But I had test for WBC and CD4 after 15th week (on 5th FEB’2014), which was WBC-7200, CD4-436. Is it the symptoms of HIV infection ? I am living with my family, but from the day of exposure to till, I have not any sex(Oral/Viginal/anal) with my wife. But her RA factor found positive in one month back, is it any way related to the exposure ? But I am sharing same bed/utensils/toilet/comb/towel/wash basin/car/TV remote/nobs/handle/book etc. Is there any chance to get infected of my family members ? 1. Do Salvia/tear/urine/spite/vomating/nosal waste have hiv virus or able to transmit the same ? 2. How many time/duration this hiv virus survive in open air at normal temp ? Request to advice pl again.

Brief Answer: Body secretions can not transmit HIV Detailed Answer: Hello, Welcome and thanks for posting your query to the forum. I can understand your concern for the possibility of HIV infection and related issues after the unprotected oral sex. After thoroughly reading your query and analyzing the complete situation with diligence let me first reassure you that there are least/ Nil chances of HIV infection in yourself. There is a scientific reason for the above statement. To tell you simply one thing is that you do not known the status of the other person. So there are 50 % chances of her negative status. Even if we consider her to be HIV postie then scientifically and medically HIV virus can not spread by oral sex. There are least of chances only when both the partners have XXXXXXX blood discharge from a cut wound that get mixed in adequate amount. In our mouth there are certain enzymes which makes the HIV virus inactivated. In my clinical experience with your kind of exposure, I have not observed HIV positivity. I quite often consult your kind of condition and never seen positive cases. Using condoms during oral sex also provided the best protection. SO this part of thing is also quite relaxing that you used condoms at the time of oral sex by female partner. Now coming on your queries : - Saliva/tear/urine/spits/vomiting/nasal waste do not have HIV virus and therefore not able to transmit the same. If at all present then the concentration is so low that it is not able to transmit the infection. There is always a term used in these cases- known as Infective Dose. This Infective Dose means number of microbial agent or viruses present in any material say for blood or semen or related things. In any of body secretions such as Saliva/tear/urine/spits/vomiting/nasal waste, either it is not present or if present then in very very minute amount which is not able to pass infection. Even sharing anything like combs etc. also do not transmit HIV virus. So the same above thing is also applicable to you. By oral sex, or bathing with anyone can not pass HIV. If such a thing have had happened then half of this world would have contracted HIV infection. HIV is such a friable and sensitive virus that it becomes dead as soon as it comes in contact with air. Rest of all the symptoms which you have enumerated are due to stress and the cold sore only and are not exactly the symptoms of early HIV infection. Now it is my personal suggestion to you that rather taking so much of tensions regarding HIV, you should simply go for HIV testing which in my view would be coming negative and therefore will relieve all your tensions. It will not only relax you but also you will be able to behave normally and have normal relationship with your wife. Regards, Dr Sanjay Kumar Kanodia (MD- Dermatology & Sexually Transmitted Diseases)

Brief Answer: Be brave for testing and it will be negative Detailed Answer: Hello, Thanks for your follow up and further queries. Regarding your further queries: 1. Why swollen lymph nodes / gland under jaw line with pain in neck/shoulders/thigh are persisting since last four five month. @ Before you think that these are swollen lymph nodes / gland, let me ask you whether you have checked with a doctor that these are frankly swollen lymph nodes. I quite often see persons with your kind of observation and after reading so much on internet, the first thing they observe is the swollen lymph nodes. Actually speaking when I examine them thoroughly then can see that the structures are simple muscles and bony prominences rather than lymph nodes only. Even if consider that these are lymph nodes then also the swelling under jaw line can be due to simple infections. Rather than thinking of HIV there can be multiple of simple infections which can cause mild to moderate swelling. 2. Why burning/pain/tingling and inflammation are persisting in mouth & tongue last four five month. @ burning/pain/tingling and inflammation in mouth and tongue are not at all specific symptoms of HIV. It can be due to severe anxiety states that we start feeling so many of vague symptoms including above. 3. Feeling weaknes and dullness all the time. @ This also is a sign of severe anxiety rather than HIV infection. In my clinical practice I quite often see your kind of status where the person presumes that all the symptoms are related to HIV. But on clinical examination as well as on laboratory testing we can confirm that the patient is totally clear of HIV. 4. I am living with my family, but from the day of exposure to till, I have not any sex(Oral/Viginal/anal) with my wife. But her RA factor found positive in one month back, is it any way related to the exposure ? @ RA factor has no relation with HIV. As I said HIV can spread through blood contact only. There si minimum amount of blood required to spread the infection. It can not spread by contact or any other body fluid except semen and vaginal fluid and that too mixing with blood. So this is totally coincidental that your wife has positive RA factor. 5. Why I have gained weight 5kg during last five month and having appetite more than normal. @ Dear sir, it is weight loss rather than gaining weight which is actually seen in HIV. Increase in appetite or weight are not at all sign of HIV. 6. Without getting test can I live a normal life with my family, kids and parent ? Can I resume sex with my wife ? @You are quite learned man, you can live a normal life without testing but then every minute you would have a fear of HIV. How can you do such big injustice with you as well your family life. Why you want to prolong a simple situation. As I said I am quite confident that your test would be negative, so a simple Tridot test will relieve all your tensions within few minutes. There is no need to go for any big tastings or procedures. In our surgical procedure we routinely do HIV testing. SO by going for testing is not at all harm. In my opinion you should be brave enough to face the situation.
