Question : I have an AMH level of 5.8 ... well that was back in March when I was tested ... Im 28 years old. Can someone please tell me how many fertile years I approx have? My ovaries each month produce in excess of 20 eggs... one month the right ovary will have more than 12-16 eggs (polycystic) with the left ovary forming 1 or 2 eggs and the next month it reverses, there doesnt appear to be a dominant ovary...Also I do not have PCOS as no other symptoms to suggest this. All day 21 bloods over the past years have confirmed ovulation has occured and all over hormone levels FSH always between 4.0 and 5.0 but LH always presents at a level on the lower end of the range but Protesterone levels confirm ovulation occured. I cant seem to fall pregnant...clomid never helped either. What could be causing my infertility? Uterus is perfect. But I have noticed that at the end of my cervix to the right I can feel a hard egg shaped lump that feels about 2cm in diameter, could be more but my middle finger cant reach any further.) The lump feels like its apart of something that is supposed to be in my pelvic region but for some reason is hanging down into my cervix region. I should also mention that when I yell or run I wee my pants. My bladder just gives way and urine just gushes out. Its rather embarrassing as Im 28 years old. Insights?
Brief Answer:
HSG and semen analysis required
Detailed Answer:
Hi,
I understand your concerns about infertility.
Following is my reply:
1) You have good follicular reserve with PCO.
2) It is very difficult to predict how many more years of fertility left as fertility also depends upon quality of eggs and not numbers alone.
3) Since you have not conceived with good ovarian reserve, please undergo HSG ( Hysterosalpingogram) test to identify if fallopian tubes are patent
4) Also get your husband's semen analysis done.
Let me know if you need anymore help.
Regards

What does your experience tell you about someone who has a low but good ovarian reserve? I mean AMH of 5.8 is extremely low for someone my age so at what rate does it deplete a year on average? And what affects the rate at which it depletes? How many eggs are left whdn a female reaches a level as low as 5.8 total? A million left? $10,000? Could you elaborate please? As if I produce 100 eggs a year than I could work out years left approx...I thought the whole point of an AMH test is to determine how long a woman has left to conceive?
Brief Answer:
AMH can predict present egg pool only
Detailed Answer:
Hi,
AMH predicts no. of oocyte pool at present. Your AMH 5.8 is excellent. It is not low in any way.
AMH does not predict rate of follicular loss in each cycle nor it says how many will remain at the end of particular number of years. It can only state present pool of eggs.
AMH does not predict quality of those egg now or in future. It can only predict number of eggs.
I hope your doubt is cleared now.

I thought I might clear up I have an AMH level of 5.8 pmol/l.
I live in Australia. So 5.8 pmol/l is that low?

Brief Answer:
Good reserve
Detailed Answer:
Hi,
5.8 pmol /L is equal to approximately 13 ng/ml.
This is suggestive of PCOS. it's good reserve for sure.

I just used the conversion calculator from pmol/L to ng/ml and 5.8pmol/L is EQUAL to 2.58ng/ml.
Please explain? So I am extremely low?
