Does Long Term Usage Of Acitrom Have Any Effect On Future Pregnancy?

Question: My age is 25. i'm taking acitrom 3mg for last 10 years for DVT. due to long time intake of acitrom whether there will be any problem in pregnancy? what are all the problems will occur in my future?? still now there is no problem for my health condition due to this acitrom drug.. please clarify my doubt..

Brief Answer: Need to know it was Provoked or Unprovoked episode Detailed Answer: Dear Friend. Welcome to HCM. I am Dr Anshul Varshney. I have read your query in detail. I understand your concern. As a rule, DVT has to be treated for defined duration as follows: 1. Idiopathic (Unprovoked) Episode of DVT- Acitrom is given for life long. 2. Provoked episode - Acitrom is given for 3-6 Months, and causative agent is removed. By saying Idiopathic, i mean that DVT developed without any precipitating cause. By saying Provoked, i mean that DVT developed due to some precipitating factor like Prolonged bed rest, Fracture, Surgery, Long Hault travel. So, when you developed DVT, your workup must have been done to rule out the cause of DVT. I would like to see your all documents, so that i can give you a better reply that till when you need to continue DVT and what complications you can have. Regarding Pregnancy, problem can occur in pregnancy if you have some problem related to increased coagulation. APLA (Anti Phospholipid Antibody Syndrome) is the most common one complicating the pregnancy. So, i would suggest you to give me details, and attach your all investigations so that i can give you a better advise. Brief Answer: Require life time anti coagulation. Detailed Answer: Dear Friend. I have reviewed your discharge summary. This discharge diagnosis is suggestive of Idiopathic Thrombocytopenic Purpura and Lupus Anticoagulant. Means if we categorize you according to the 2 classes i discussed in my previous answer, you fall into Idiopathic or Unprovoked variety. So, you would require anti coagulation (Acitrom) life time. You also had cerebral venous thrombosis, so you also require workup for APLA syndrome. However, treatment would remain the same. As far as your pregnancy is considered, you might have no problem in conceiving but your delivery might be eventful. So, when ever you plan to get pregnant, you should consult your doctor. You should never ever stop warfarin until and unless you get any complication by it. Your INR should be maintained between 2-3. I would also advise you to get Anti Ds DNA repeated as earlier it was slightly high. It is diagnostic of SLE (Systemic Lupus Erythematosus). If you have any further query please ask me. Stay Healthy.

Dear Sir, During pregnancy if i took acitrom, it wont cause any effect on baby? or i should stop oral drug and need to put injection for entire 10 months?? please clarify my doubt..

Brief Answer: Detail follows. Detailed Answer: Dear Friend. This is totally a decision that has to be taken by your doctor, based on his/her experiences. What guidelines say and what i practise is, that if possible, warfarin/ acitrom therapy should be avoided during pregnancy. If essential, it should be avoided at least during the first trimester (because of teratogenicity) and from about 2 to 4 weeks before delivery to reduce risk of hemorrhagic complications. Unfractionated heparin or low molecular weight heparin (Injections) could be substituted when appropriate because these agents do not cross the placenta and are considered the anticoagulant drugs of choice during pregnancy If you have any further query, please ask me, else close the discussion and rate it.
