Question: Hi I need to know the details of my semen analysis report i have just received. It is as given below:-

Quantity - 4.0 ml
Color - Greyish White
Liquification Time - 35 min
Transparency - Opaque
Reaction - Alkaline

Total Count - 70 million/cumm
Actively Motile - 20%
Sluggish - 30%
Dead - 50%
Pus Cells - 8-10/HPF
RBCS - Nil
Abnormal Forms - Nil
Agglutination - Positive

Is it normal or not?

Thanks for the query.

I have reviewed your semen analysis report.

It appears that your sperm are not functioning very well because of infection to urinary and possibly sperm storage and transport system since there are many Pus cells seen, which signifies infection.

This can be reversed with care from your part. Consume 2-3 litres of water a day for a week. Eat plenty of fresh fruits and XXXXXXX leafy vegetables as they have anti-oxidant effect which reduces sperm damage.

You may require antibiotic treatment for the infection like Tablet Levofloxacin 500mg once a day for 5 days, but only by Doctor's advise under medical supervision.

You can also take Tablet Paternia once a day for a month. This tablet has powerful ingredients like Coenzyme Q to increase sperm count by preventing damage and giving strength to sperms. Avoid bringing any heated objects or fluids near the testis area as increased temperature near testis can also damage sperm.

Get repeat semen and urine test done after a month to look for improvement and if infection is eliminated.

Do consult your Urologist for proper antibiotic treatment and other required investigations as required for best results.

Is it a big problem?

I had this test about 1 year earlier also. At that time the pus cells were not present but the actively motile sperms were almost the same as they are now.

Actually, my wife is undergoing treatment for PCOS and we wanted to try for the baby now.

Will it be cured in a month or two? What other tests can be useful to know the exact problem? Thanks.

Hi there.

Kindly do specify the previous semen test values.

The problem appears to be that the sperm are not strong enough to move forwards, hence motile sperm are less. Generally motility should be above 50%. Due to the damage due to infection the sperm is moving haphazardly and weakly. Also there are many dead sperm which can be due to the infection.

The tablet Paternia has useful ingredients which limit the damage and also provides energy to the sperm. Do try this medication for a month along with the other dietary modifications as I have mentioned and repeat the semen test by then. It is possible there will be definite improvement by that time.

Your wife needs proper monitoring of her ovarian follicles during the time you wish to try for pregnancy. Ultrasound monitoring of her follicles will help you decide which day to have sexual intercourse so as to have highest chances of pregnancy with your wife. Do discuss with her Doctor in this regard.

Just being curious...

I have one question in my mind, what could be the cause of this infection? I did not have any major injury in that area? Once, I got allergy from a hair removal cream I used, there were some bruises, they got healed over the time, can it be the cause of infection? or is it related to diet?

Thanks for the query.

The infection you have is possibly urinary tract infection. It can result due to various causes like due to dehydration, improper hygiene, unprotected sexual intercourse with multiple partners, low immunity etc.

Nothing to worry. A trial of antibiotics should eliminate simple infections. Drink plenty fo water to wash away the pus cells which is the reason your sperm are weak. Eat plenty fo fresh fruits and XXXXXXX leafy vegetables everyday as they provide powerful anti-oxidants which reduce the damage to sperm and provide strength to sperm as well.
