Welcome to Healthcare Magic.
The yellowish white spoke seems to be a furuncle or plainly called a boil.
It is a local pus collection.
Ideally it should be managed by a XXXXXXX and drainage of the pus and sterile dressing by a local doctor.
You may additionally need short coarse of antibiotics.
