Question: Hello Doctor,

I'm a 32 yr old guy who's virgin. I'm going to get married in Dec of this year. I used to watch porn occasionally when I'm sexually aroused n masturbate. Everything was normal until last 4-5 months where i started having premature ejaculation while getting discharged through masturbation.

It all started gradually where during initial days i used to have hard erection but with fluid like substance discharing from penis upon being excited (possibly semen but bot sperm). I assumed that to be normal thinking bcz i'm aroused perhaps that discharge of fluids before ejacaulation is common. However, eventually i had times where even for a min if i imagine of a romantic moment then fluid starts discharging (when even no errection) which i could feel through a wet spot on my underwear. Further with passing of time when i use to watch porn i started having premature ejaculation and now i face erectile dysfunction where i'm losing that hardness in erection. All this has starting happening only since 4-5 months.

As i'm going to get married soon, i've started getting afraid of this present situation. Thoughts like how can i manage it after marriage and how will i be able to statisfy my sexual partner in bed are common now. Should i see a sexologist? If yes, should i prefer Allopathy or Homeopathy. I've come acorss several homeopathy youtube channels where homeopathic doctor claims 100%25 proven results and share success stories without any side effects. If my physical condition needs to be treated then i don't want to delay the treatment considering i will be getting married this December and do not have much time. Please advise. I appreciate any sort of help.

Brief Answer:
Regarding erection issues and premature ejaculation

Detailed Answer:
Hello and welcome to ask a doctor service..I have gone through your query and noted your concern.

Precum is what you get when sexually excited and that is pretty normal. As far as premature ejaculation and not being able to get a strong erection is concerned, I think that is primarily because of fear (performance anxiety) of not being able to have a sexually satisfying married life. It is a very common reason for people seeking consultation and this fear in fact the root of the problem. I think, you should just stay away from such thoughts because the more you worry, the more they would interfere in you getting a proper erection and the more you try to prove it to yourself, by constantly checking for your erection while masturbating, the more it affects your erection. The key to having a good sex is to have a clutter free mind. Nothing else should matter while you have sex. So, no need to try any medication. Keep the doubts away and believe that everything is normal with you because it is!

Hope I have answered your query. Let me know if you need any more assistance.

Thank you so much doc. Something that i did not mention earlier was that from past few weeks i do not feel erection while I'm asleep or wake up in morning which usually i use to have almost every day. Is it common? This bother me along with other factors that i have mentioned in my previous query. I feel this problem is gradually increasing. I have read on internet the role of testosterone in sperm production, erection and libido. Just to be double sure i wanted to get testosterone level test done. Would you please recommend me this test along with any other test which ensures that I'm physically fit? Thanks again!

Brief Answer:
regarding erection issues

Detailed Answer:
Hi.

Lack of morning erection is the first thing to happen in case of stress. So give some time as most likely it is a temporary thing and would get ok if you stop worrying. Nevertheless, if you insist, you may take a test for total testosterone and free testosterone levels for your own mental satisfaction.

Take care

Thanks doc. I feel relieved by your response. Last thing which i want to check if it's common was discharge of sperm while urinating. Often times i get small portion of sperm discharged when urinating while defecation. Is this something common or due to bodily deficiency?

Brief Answer:
regarding genital complaints

Detailed Answer:
Hi.

That needs to be confirmed first by urine analysis whether it is semen/ sperm or just normal urethral secretions. Even if it is semen/ sperm leakage it is not a cause for concern, as some semen can remain in urethra after ejaculation and flows out while urinating. However, not all discharge from the penis is semen, so it needs to be first confirmed whether it is actually semen or just urethral discharge.

Doctor, I have metioned all that i was going through in a series of queries as I wanted them to discuss with a medical professional and seek advise. You have been very patient to read them and reply all my queries. Thanks a lot. Really appreciate it.

I would like to inform you that with some basic dietiary changes i am now having morning erections (and hard ones) and I plan to continue to follow this diet. I also plan to get the testoesterone levels checked before my marriage just to ensure i carry right level. Lastly, will you recommend any medical tests or seeing a urologist or sexologist for urine analysis? Thanks!
