Is ORIF Appropriate For Left Radius Fracture?

Question: Hi,

This 48 years old female member presented to the hospital after an accidental fall at home and injuring her left wrist. She presented with pain and swelling in the left wrist. X-Rays were done and revealed Left Radius Fracture

On Examination:
Patient was distressed with pain and swelling in the left wrist

X-Ray Left Wrist:
     Comminuted Fracture of left distal radius is identified
     Fracture line is reaching the articular surface
     Butterfly fracture is noted at the radial aspect
     Dorsal angulation of the distal fractured fragment is identified carrying the wrist joint with it resulting in dinner fork deformity
     Fracture of ulnar process is also noted

Diagnosis:
Left Comminuted Radius and Ulnar Fracture

As per the above findings will ORIF be done as an emergency or a cast should be applied and treated electively. (as per the guidelines) and why?

I have gone through your query and understand your concern. There are three options. One is close reduction and hold by external fixation or per cutaneous K wire fixation. It is generally most accepted method.
Other option is close reduction and cast application but in this method late collapse and malunion is comman.
Third method is open reduction and internal fixation.
Supporting of fracture by slab/ cast should be carried out as emergency procedure to reduce pain of fracture. But internal or external fixation is semi emergency or planned procedure.
Choice of procedure depends on fracture type, patients profile and surgeons choice and expertise. I personally favour close reduction and fixation by K wires or JESS external fixator. I can serve you better if you upload x Rays.
