Dosing of PPI depends upon indication of its usage, specially in endoscopy proven ulcers it is given twice daily with 40mg dosages but in normal GERD and gastritis symptoms it is usually given as 40 mg once daily before meal.
In prolong usage with high dosages it may cause achlorhydria, increased susceptibility to infections as blocking acid for long removes acid barrier against bacteria which otherwise helps to protect against infections.
