Question : The tip of my penis has a slight cut at the slit and round it is red, looks like a rash, and is very sensitive

I have reviewed the attached photograph and it seems you have developed yeast infection.
If I was your treating doctor I would recommend you to apply clotrimazole cream on the affected area twice daily for 2 weeks besides maintaining proper personal hygiene.
