I understand your concerns but don't worry. Cold extremities can occur with high core body temperature while hydrating. I would recommend you to feel for peripheral pulses I.e. radial pulse in both hands and posterior tibial on back of medial malleolus on both legs. If present, then just use anti pyretics such as Tylenol or Meftal P. You may repeat the dose every 8 hours if required.
If you feel that the peripheral pulses are feeble or not palpable at all, then visit a pediatrician. This is important to rule out cold shock associated with many infections.
