Suggest Remedy For Bleeding And Itching Caused By Internal Hemorrhoids

Question: I am a 43 yr old male, 5'8" and a stocky build at 250lbs. Over two years now I've had what I believe is internal hemorrhoids which seem to come and go. Extreme itching and white odorless film around the anus constantly. The skin is very thin and bleeds often. I've also developed numerous pimple or cyst type bumps that come and go every few days. They are usually deep and do not break the skin. Some do and are quite painful and bleed. When I first started having these symptoms the cysts did not come to a head but revealed themselves like a lesion with a very fine crust cover. They no longer crust and when they surface now they are as already stated above. Any advice will help. Thanks
I realize how difficult it must be to properly diagnos online. I don't believe I have a fistula because the symptoms clear up then return. I believe fistulas don't act in this way. An abscess is a strong possibility. I don't believe I have cystic lesions but rather some type of boils or pimples . I'm wondering about something more along the lines of leaky gut, upset flora or yeast infection. My thoughts are in this range. I have recently noticed that my stool seems to be filled with undigested food and I have an insatiable appetite. Thanks again

I have gone through your query and I have noted down your concern.
Intertrigo- inflammation of body folds- is a likely possibility. Intertrigo develops when body folds trap moisture and allow candida / yeast to proliferate causing symptoms like itching, redness.
Underlying conditions like haemorrhoids increase the probablity of developing perianal intertrigo because it encourages skin irritation and moistness (due to discharge) of perianal skin. Being overweight is also a predisposing factor for intertrigo.
The area of intertrigo might subsequently develop recurrent secondary bacterial infection i.e boils and furuncles.
If I was the treating doctor I would suggest a course of an Oral antibiotic e.g Amoxicillin Or Cephalexin for a week, to settle down boils/ pimples.
In addition I would suggest weekly/ once every week dose of an Oral antifungal e.g Fluconazole 150 mg, for 4 weeks.
Topically you may use an antifungal dusting powder e.g clotrimazole dusting powder. This would keep the area dry and thus would discourage yeast growth in addition to its antifungal action.
A tablet of cetrizine 10 mg once daily would provied you symptomatic relief from itching.
