Brief Answer:
May be Malgaigne's bulge, may be hernia.

Detailed Answer:
Hi.
Thanks for your query albeit short and specific history.

To recapitulate: male/60 visited Doctor at ER and told that this is not a hernia.
Lump comes and goes with lifting and exercise at work, on the left side of the pelvis and above groin.

By definition:
A true hernia is protrusion of the internal contents out of its normal confines, through an opening called a neck and can remain outside the confines, comes out on straining through a sac and always reducible (unless complicated by obstruction)
Another type is called Malgaigne bulging wherein there is no true neck or sac it is overall weakening of the abdominal wall and occurs exactly at the spot you have mentioned, hence it is possible that the Doctor at ER would have not been believing this to be hernia.

I would advise you to take an opinion of a General Surgeon to confirm what it is and go for definite treatment if needed.

I hope this answer helps you, please feel free to discuss further if you need to or if you feel that there is a gap of communication.
