Suggest Remedy For Pain And Retraction Of Testicles

Question : I have just recently had acute onset of scrotal and penile issues. My right testical retracted inside me. I pushed in back into the sack, but it continued to ride high like at any moment it could go back in the groin area. Therefore, I constantly was gently tugging it downward to prevent such a thing from happening. A couple days later, my sac became really loose and lost its elasticity. Then, a couple days after that, I started to develope enlarged veins in my sac. Theses veins were mainly on the left side, but some toward the back of the right side, as well. I went to a urologist and said I may have a left side varicocele. However, due to the acute nature and all the other stuff going on, I fear it is much more serious than a little varicocele. Additionally, since his evaluation, I have had numerous new enlarged veins appear on my actual penis shaft that were never there before. Also, I have had pretty constant and significant pan in my testicals (it has improved the last day). Finally, the left testical seems to have atrophied over the last two days to a noticeable degree, via my own elementary palpitation.
Do you know what could be causing all of these disturbing issues. I have had many anxiety fits over this and I feel devastated and full of doom and gloom.
Side note: I have been being treated for lymes disease and thus all started after I had been on Plaquenil for a short time. I have since stopped that medication. And have not taken it in about a week.
Thank so much for your response, as I am in a state of disbelief and torment over this development. I'm 24 years old.

Your varicocele isn't severe. Testis don't atrophy in few days.

Hello XXXXXXX and welcome to HCM.
As an Urologist, i fully understand your concern and anxiety.
However, let me assure you, there's absolutely no reason to feel so anxious.
The reason being, veins over the penis and scrotum, are commonly seen in many males, and become prominent after masturbation or sex.
You should avoid trying to tug at your testis, as this will increase your pain. You can gently push it downwards, whenever it moves upwards.Inform your doctor.
Atrophy of testis can't occur in few days. So don't keep checking it all the time.
In your case, the Urologist wants an ultrasound scrotal scan, to confirm, if there's
a varicocele. This means it's not not severe, as in many people. In some, one can see a prominent varicocele, and scan may not be required.
Also, Lymes disease or Plaquenil, are not related to your symptoms.

The aid for your reply. I do have some additional information and some follow up questions and concerns.
1. Plaquenil can cause kidney problems and muscle spasm and since the renal vein goes from that kidney area, I wonder if it could have cause something. Also, I have never had any issues with testiculr or penis malfunction or vein issues. I am extremely athletic and in great shape (other than Lyme treatment). The acute on set of this makes it hard for me to believe the medicine was not a causal agent.
2. Several days ago I also experienced painful electrical shocks/spasms that extended from my right testical (the one that retracted temporarily and now sits higher than it did previously) to my right abdominal area, about liver heights it a little higher. Does this mean anything to you?
3. You mentioned that these enlarged penile veins occur during enlargement of the penis, sex, masturbation, etc. however, this is not what in dealing with. These newly enlarged veins that pops up over a two day span are present even with the penis fully flaccid. I keep a close check on my genital area, and these now very visiblly enlarged veins have never been visibly present (obviously they were there under the skin but we're much smaller and therefore not visible) throughout my 24 years on this earth. They appeared seemingly overnight one night and the next night another two appeared. What I am concerned about is that some factor is causing extra pressure to my genital vascular system, which is causing enlarged veins. I did not have prior enlargement of testicular veins either and they too popped up over night and then grew some over the next couple days. Could I have a blood clot or pinched vein upstream?
4. You stated that testicular atrophy would not occur in a matter of days. This may be true in 99% of cases, however, my left testical (the one with the most enlarged veins) has definitely shrunk over the last two days. It has also become slightly more soft that the right testical.
I understand that you replied to my question with answers that fit most all cases. However, my case is not most all cases and does not fit the traditional mold. Please take that into consideration when addressing each of my points (1-4) in your reply. Remember, my cases was acute onset, possibly medicine agent causal (it gets in deep tissue and can cause skin and muscle spasming issues), and seems to be continuing to progress. It also involves the penis as well as the testicals. As you can see, this is not the traditional varicocele or group of varicocele a that came up over time and are now causing discomfort, etc. my case is much more in depth and much more serious than that.
Please address each of my issues (1-4) above in detail and with an open mind and maybe some outside the box thinking. I understand that you are the expert here, but through all of my Lyme treatment and diagnosis trail I have learned that in many cases, the traditional medical answer to a situation is completely amiss and can cause much damage because the doctor relied on prior instruction and traditional case theory, instead of deeply assessing each case in detail and with an open medical mindset. I hope I do not corn across offensive, as I am sure you are an incredible physician. I home you understand that I am simply very worried about this issue and want to make you understand where u am coming from as you answer these questions. By the way, I am no quack of a person out here imagining issues. Ha I went to Wake Forest University and was I the top 15% of my class. I will attend law school next year. My issues are real. I am so thankful for your time and let me know if there is anything else I can do to thank you for your help.

Don't worry about Lymes disease, as it's cured with treatment.
1 Plaquenil isn't known to cause kidney problems. So, don't worry about that.
2.Testis retracting upwards can cause pain, which is relieved after lowering it.
Liver is in the right rib cage, so testis, obviously can't migrate so high.
3.If you go back to my answer,the veins become more prominent in masturbation.
As you're athletic, the veins also will be larger in size. Blood clot in the vein,
present in a different manner, which you don't have. Be relaxed.
4.I'm re-assuring you, that testicular atrophy would not have occurred, after the
Urologist examined you. It can be confirmed by a doctor after examination only.

Sorry to keep pestering you, but I do have a few follow ups.
1. You said that testies can hurt when retracted and pin relieved when they are lowered. However, mine is not lowering, as it is higher in the sac than normal. So why is it not lowering? AND why did it raise to a higher position in the first place - this is where my testiculr torsion concern came in, as I read it can cause the testical to be higher up and painful (which mine is)?
2. You say the liver is in the right rib cage so the testical annoy migrate so high. I agree. However, I didn't state that I thought the testical was going to or did migrate that high. At the high retracted point, it was just barely above the sac area. What I did say is that I got electrical shock/muscle spasm feelings that traveled from my right testical up to a point around my liver in my right abdominal region. I'm not concerned that the testical will retract up that high, I haut wonder why I was having those spasm/shocks and if it could be related to my current testicular and penile issues.
3. You state that penile veins become more enlarged while masturbation or if athletic. One, I no more athletic now than I ever have been. My physical fitness or regime has not recently increased, although my penis veins have. Also, I stated that my penis veins were engorged even in a flaccid penis state and that I have NOT had sex masturbated in the last several days, so this could not be the cause of the enlarged veins either. So neither of the causes of enlarged veins you stated could at all be applicable in my case. So what is the cause or possible causes of this? This is where my blood clot of pinched vein upstream thought came from.
4. You stated that testicular atrophy could not have occurred in the 4-5 days since my urological exam. I am NOT saying that my left testical has shrunk to a pea size, but I AM saying that it has shrunk slightly over the last several days. Also, it is softer than the left testical. Even if you still do not accept this as truth (although I assure you it is), could you tell me what COULD cause such an effect?
Please address all of my questions and each pet inside each question. Hair want to make sure I get a thorough response. I understand I a just one orient to you, but please realize that this is my everything. I only have one penis and one set of testicals to go through this life with. I'm not trying to be funny...I am deeply distressed/concerned. Believe me, there is a significant issue going on in my genital area.
Again, thanks you for you tie and concern. And don't hold back being truthful with me, in fear that I will be too alarmed. I'd rather deal with it now than find out later.

You're welcome with as many questions as you please. No pestering at all.
1.There's a condition called retractile testis, where the testis can go move up and down. If it's causing frequent pain, surgical treatment may be done to fix the testis in the normal position. That, the Urologist will decide after examination.
2. The pain feeling going right up, is due to pain being conducted up by nerves.
3.Variable sizes of veins are seen on the genitalia. Don't try to think of clots in veins. You don't have any of it's symptoms.
4.What you're describing as atrophy, is a subjective feeling. Urologists have an objective, accurate way to assess atrophy, after examination and scrotal scanning.
