Pain on left side that runs up and down side. Pain has been present for 4+ months. Sometimes its very painful, other times the pain is there but not severe. Pain in the side seems to be made worse when he eats.

His symptoms are for sure not caused by one issue.
Left side pain can be caused by peptic ulcer, urinary tract infection or even large intestine tumor. I do not think alcoholism history has anything to do with his current symptoms. Alcoholism usually leads to liver changes and pancreas changes. It may result with liver cirrhosis (which does not give any pain) and chronic pancreas inflammation (gives upper middle pain which can radiate to the back, fatty diarrhea, weight loss).
Blood in the stool can be related to the pain, but it has nothing to do with testis pain and urinating problem.
It is important to determine the source of this bleeding. If the blood is on the stool surface, not mixed with it and is bright red, hemorrhoids are the most probable reason.
If the blood is mixed with the stool, but not black, large intestine tumor can be the case.
