default
Posted on Mon, 21 Dec 2015
Question: Hi I have been have redness and irritation at the base of penis near my pubes. This has been going on for a couple days now and I have noticed white creases that appear to be raised. Any Ideas what this could be? Infection? Herpes?

I should note that I am sexually active.I have attached the pictures. The irritation is worst on the right side (where white creases appear to be)

Brief Answer:
Need some more information from you sir, thank you

Detailed Answer:
Hello sir and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. I have a few questions before I can comment or even assume the cause for your presentation; if you would be so kind as to answer each question of mine it would greatly help sir:

+Are you sexually active? If yes, when was the last time you had intercourse?
+If you did have intercourse, was your partner a known person? Someone you have had sexual relations with before?
+Have you left your genitals wet after sexual intercourse, oral sex or masturbation? Meaning, did you not wash your genitals off?
+Is it hot where you live? If yes, do you sweat often?
+How would you rate your personal hygiene on a scale of 0 to 5; 0 being extremely poor and 5 being obsessive?
+How often do you shower?
+Did this presentation come about after you removed hair from the region?

I look forward to hearing from you in order to help you further.

Best wishes.

I had sex about a week ago and did not clean the region. But I did notice the white mark in the region before I had sex. Also I do not know if the marks existed before or after I removed the hair.
Also I was with another person, not known, but we did not have sex but I did have itching in the creases of my crotch for a couple weeks after.

I do not live in a warm area but I do sweat quite a bit and I shower daily.
I should note the itch in my crotch creases has subsided since I have consistently jock itch cream

Brief Answer:
Penile candidiasis, will respond to clotrimazole

Detailed Answer:
Hello once again sir.

Thank you for your response.

It does seem to me like a yeast/fungal infection, and so please do try and follow the following suggestions:

+Try to get your hands on clotrimazole cream to be applied to the affected (clean and dry) twice daily for 10 days
+Always keep the region dry
+If you sweat much, then carry an extra change of inner wear which you can change around mid-day
+Avoid masturbation and sexual intercourse for the next 2 weeks at least
+Make sure you shower everyday and clean and dry the region well enough after

I am sure you shall be perfectly fine in no time. Please feel free to write back to me for any further clarifications, I am always here to help.

Best wishes.

Okay thank you for your help. Also just to help my conscious you do not think there is a chance this could be any sort of std? I was tested 4 weeks after the incident and everything was negative but I know that is not a good time frame for all diseases. Your insight would be very helpful and comforting. Thanks

Brief Answer:
Not with how much I know and from the images, nope :)

Detailed Answer:
Hi sir.

It does not look to me like any STD, and with the given information (you would have mentioned the symptoms if you had them), nothing indicates or points towards any known STD. So rest assured and always play safe.

Best wishes. :)

Thank you. Appreciate your helpHi, sorry for the numerous questions, but I have another question to ask.
The redness around the base of my penis, when can I expect it to go away?
I have attached two more pictures for a better understanding of the irritation. And can this be passed through protected sex?

Brief Answer:
About 10 days on the treatment sir; mostly no, but it can at times

Detailed Answer:
Hello once again sir.

Please do not aplogise for asking me a question, I am here to answer any/all questions you have so please do not hesitate to write to me.

The redness is minimal sir, and if you truly do have candidiasis then the clotrimazole will work wonders and you should be perfectly fine within 10 days time.

But no sexual intercourse or masturbation during the course of treatment and none for at least a week after the treatment is complete.

If it is indeed a yeast infection, the cause is leaving the region moist and unclean which allows the yeast/fungus to thrive in those surroundings. But if one partner has it and you involve in sexual intercourse and do not clean and dry up adequately, the other can also get it; although it is nothing serious. I hope this helps sir. Wishing you a rapid and complete recovery.

Best wishes :)

Hi, I have been using jock itch cream instead of clotrimazole. Is there any difference between the two or should I be using one or the other.
I am just curious because today the base of my penis where it meets my body is more red and periodically more itchy than usual.

Thanks again.

Brief Answer:
Clotrimazole prefered over miconazole (jock itch cream)

Detailed Answer:
Hello once again sir.

Jock itch creams usually contain miconazole, which is mostly used to treat a jock itch and also ringworm infections.

I would recommend you shift over to clotrimazole twice daily for the next 10-14 days, and report to me in a few days about how you feel and whether or not there is an improvement sir.

Best wishes.

Hi, i have noticed around the redness at the base of my pubic area that there are what look to be two thick white lines. The lines do not hurt or itch for the most part and are about a little smaller than a 1/4 inch. Could these be apart of the infection, infected hairs, or an std?

Sorry I may sound paranoid, but I am just trying to sooth my conscious.
(also Let me know if a picture will help)

Brief Answer:
Nothing to worry about sir, it will subside with the infection :)

Detailed Answer:
Hello once again sir.

My sincere apologies for the delay as I had a 33-hour shift, and it was quite excruciating.

The lines that you noticed are just skin folds that have changed colour a little simply because of the infection, and for no other reason at all. So do not worry, I see that you are mildly paranoid but do not worry. Have you purchased all the advised medications yet? If yes, how many days since you have been using them?

Best wishes sir.

I was using jock itch cream for four days consecutively then I as of yesterday I switched to the clotrimazole like you said. Also a little red spot has appeared in near the area by the base of my penis. Should I be worried?
doctor
Answered by Dr. Shoaib Khan (8 hours later)
Brief Answer:
Do not worry sir, let at least 5 days on clotrimazole pass

Detailed Answer:
Hello once again sir.

Let us wait for the clotrimazole to start taking action, it should be used twice daily over a clean and dry region for 10 days. You should start experiencing an improvement in about 4-5 days time, so report to me then. And if there is no improvement then, we shall make changes and I shall guide you from there.

Keep the region dry as much as possible, use the cream religiously and stay calm. Send me a picture of the red spot in your free time.

Best wishes.

Here is a picture of the red spot. Also the itching has gone to burning at the base and mid section of my pubic area, but the redness has disappeared. Is the burning a result of the cream working?
doctor
Answered by Dr. Shoaib Khan (6 hours later)
Brief Answer:
Lets hope you start feeling better in a few days time

Detailed Answer:
Hi sir.

The latest picture also resembles a presentation expected with the infection. So nothing new there sir, if the treatment works then this should also subside.

As for the burning, it is not an indication of the cream working exactly, but is expected once the treatment is initiated, don't worry sir it should subside soon.

Best wishes sir.
