I have gone through the details but am afraid that I did not understand what you meant by HFpEG although I assume that afib means atrial fibrillation. It would be preferable if you use the expanded terms in order to avoid confusion.
The fever could be due to a viral infection and irrespective of the use of antibiotics, it would last for 5-10 days. But if it is already 10 days as you have mentioned, you should visit the doctor and run a couple of blood tests, especially to rule out typhoid fever if that is prevalent in your area of residence.
About the pain that you mention, the first thing to get done is an electrocardiogram (EKG) to evaluate if the pain is indeed due to heart cause. Even if I assume it to be due to hyperacidity, I do not think that seltzer water is the solution that you are looking for. Rather, I would suggest taking small but frequent meals and to take Prilosec (omeprazole) (available over the counter) once daily in the morning. Increase your total fluid intake. Since you mention that you have constant cough, I think that the pain could also have a lung cause. Taking steam inhalation should provide relief. In any case a proper examination by a doctor is essential and I would strongly suggest that you visit a doctor without delay.
