default
Posted on Mon, 7 Dec 2015
Question: Been having some issues with heart/chest related pain and need second opinion:

- after eating a meal I always get sweaty palms within 30-60 minutes, if I walk excessively after I always get palpitations. This used to give me sharp chest pain but now has subsided to sweaty palms and the palpitations.

- I occasionally get pinching pain from heart/chest area when lying down mainly on left side

- two months ago this used to be daily sharp chest pain but now is mainly the pinching pain in chest, maybe related

- get daily palpitations, some of them are soft and not really bothersome, some are strong in nature

- sometimes laying on left side about to fall asleep I feel the pinching sharp pain and it feels as if I will become unconscious

Brief Answer:
In most probability there is no Cardiac problem. Be reassured

Detailed Answer:
Hello , I am Dr. Mody and I would be addressing your concern .
Considering your age and all the investigation you have already done , let me reassure you that there is no underlying cardiac disease .
Two things which comes to me :
1) in 2 d echo look for the status of mitral valve , is there any prolapse, as mild mitral valve prolapse may cause the symptoms you described at your age
2) If you have weak stomach, have heart burns particularly after a heavy meal , burping sometimes, when you lie down after food , consider reflux gastritis and consider taking Tab . Neksium 40 mg 15 mins before breakfast for 15 mins For three weeks
3) if you smoke , STOP . Less coffee , chocolates, Spicy food , citrus fruits .
4) if overweight consider losing weight
Let me know how you feel after 2 weeks .
Regards Dr Priyank Mody .

Hi Doctor,

Thanks for your response. After many ECG's, blood work, Echo, and also two holter monitor - second one results should come next week all was normal but also had many palpitations after meals, I will see what second monitor says when I get results.

I have had constantly slightly elevated Monocytes (0.9 now - highest was 1.2) , and d-dimer was elevated before, the d-dimer was repeated and lowered. Could this perhaps stem from a viral infection that is trying to go away too?

The Echo results were normal:
Cardiac measurements were normal
Ventricular Function were normal
Valve assessment -
Mitral Valve structure normal, no regurgitation
Aortic valve structure normal, no regurgitation
Tricuspid valve structure normal, no regurgitation

This test was done a month ago but could this change in a month time or come and go? At the time of the Echo it did not happen.

Not overweight for my age, and previously smoked Marijuana but quit beginning of September been two months.

I did notice when I go for a brisk walk and afterwards it feels like a gurgle sensation by the heart, not sure if this is related or maybe I am looking for any problem too. I also notice when I walk it feels like a pinching sensation sometimes in legs only lasts for less than a second too, maybe nerve or circulation?

Thanks for your advice.

Brief Answer:
It's not the heart :)

Detailed Answer:
Hello ,
As I start I would like to reassure you about everything being fine and you are looking for a problem which might not be there .
Coming to your questions .
No echo doesn't change much in a months so no need to repeat.
Mostly 2nd Holter should come normal too .
D dimer may increase due to a viral infection , and the amount of increase is important. Also if serial measurement is normal don't worry about the same .
Rarely for any acute infection monocyte may go high , however they will not result in any of your symptoms.
Many a times if you are have quitted marijuana, these may be subject signs of withdrawal. Also if you have been taking it for long time it may just weaken your nerves giving rise too all the funny symptoms you describe.
I would still suggest tab neksium 40mg for some days and take a multivitamin which contains b12 , b complex , vit d and other microelements if you are a junk food eater .
Let me know if any other doubts , I would be happy to help .
Regards Dr Priyank Mody
