Nursing diagnosis: Intracranial hemorrhage (brain hemorrhage)
Priority assessment would include:
Assessment of consciousness- Glasgow coma scale (GCS)
Blood pressure, pulse rate, respiratory rate,
Temperature
Assessment of swallowing- by doing swallow test
Assessment of motor power (of limbs)
Interventions may include:
1. Change of patient position every two hours,
2. Placement of Ryle's (nasogastric tube) if swallowing is inadequate
3. Feeding through the Ryle's tube,
4. Administering BP and other medications,
5. Assisting in physiotherapy,
6. Deep vein thrombosis (DVT) prophylaxis by using stockings, mechanical compression, etc
7. Psychological support to patient and family
