Question : I'm patient of psychiatry since last 10 years. At first I used to take different types of medicine related with Ocd diagnosis prescribed by Dr. XXXXXXX XXXXX, XXXXXXX Name of medicine are Ddpramine50,zenoxa od 300,fostera5, 5htpn50, lyvit.angical and angizyme. Nowadays I am taking two tab of ddpramine 10 after meals according to situation and suggestion of doctor. But my mental condition gradually relapse. Excessive worries, obsessive tendency, sadness of mood, fear of harming self and others, decreases sleep, thinking unwanted thought stressing's me lot, restlessness, feeling nasty harmful thoughts . confusion about myself -doubting in natural thing, thinking life is meaningless. Is the cause of lower strength of medication.? What to do Sir at present condition? Is there any way to resolve my problem. Should have I change of medication.? Actually I'm not willing to take any kind of medicine.

I read your query and understand your concerns.

From the available description it appears that you responded well to initial treatment for OCD by Dr Banarjee and slowly the dosage of medication reduced to make least possible medications. The current symptoms, as you commented, clearly indicates the relapse of Obsessive compulsive disorder with secondary depression.

We need to understand that Ddpramine 20 mg per day is quite low for the symptoms and I feel there is need to increase dose to atleast 50 mg (original effective dose). The other option is to use Fluvoxamine 50-100 mg after discussing with your doctor. In nutshell there is definite need to increase the dose of medication for control of symptoms in current situation.

The another option is use of psychotherapy. In the OCD exposure and response prevention a kind of cognitive behavior psychotherapy is quite effective. I do not recommend its use in current stage but once there is control over symptoms, it can be used provided it is available in your city.
