Question: Hello, I have a history of gut problems. IBS, once Giardia? Sometime early spring late winter a new pain started. Instead of my usual area for gut pain it is in the lower left quadrant of my gut. Is was off and on but now it almost all the time. During the day it is usually mild but by night it worst. It feels like someone has grabbed my gut. It can make sleep impossible. It is now worst some mornings too. I am on a waiting list for a GI DR but it getting to be a long wait. I also have a ovarian cyst but no systems but this pain. I am very bloated and continue to gain wt.

Pain in the lower left side of the abdomen is called left lower quadrant pain. It is often related to the digestive tract, but can also be related to conditions of the body wall, blood vessels, urinary tract, or reproductive organs.

Going through your history it sounds more like possible diverticulitis.Diverticula are small, bulging pouches that can form in the lining of your digestive system. They are found most often in the lower part of the large intestine (left lower quadrant).Sometimes due to infection by bacteria or protozoa one or more of the pouches become infected. That condition is known as diverticulitis.

Classical presentation of a case of diverticulitis is-" Pain, which may be constant and persist for several days or can be off and on felt in the lower left side of the abdomen.

Tests suggested initially are-
Fecal calprotectin-it is s a biochemical measurement of the protein calprotectin in the stool. Elevated faecal calprotectin indicates the migration of neutrophils to the intestinal mucosa, which occurs during intestinal inflammation.

CT scan of abdomen-Reveals inflammation in diverticula and would even rule out any other pathology.

Medications adviced are-
Take Tylenol twice daily for pain and inflammation.
Antispasmodic like Dicyclomine 10 mg twice daily can be taken to relieve spasms(Prescription med).
Take a probiotic like VSL#3 thrice daily.It will improve overall digestion.
Once confirmed that its diverticulitis therapy with antibiotic like Azithromycin and antiprotozoal like metronidazole would be given for 14 days.
Take tab Nexium 40mg twice daily to provide relief in bloating.
Take Fybrogel husk 2tsf twice daily.This will help to increase bulk of stool and will add to your fiber intake.

Along with it food habits should be changed-
Take simple organic and vegetarian home cooked low fat food for few days.
Avoid alcohol,smoking aerated beverages,junk food like pizza burger etc.
Take lots of prebiotic rich food like yoghurt.
Increase your fiber intake take oats,wheat,barley,vegetable and fruit like Pear,apple and oranges.


Weight gain can be due to hormonal inbalance also( ovarian cyst or due to inadequately controllled thyroid gland.)
A thyroid profile and serum estrogen and progestrone level should be done for that.

I have taken Linzess from free samples it help my chronic constipation, IBS, from the DR. it helped the constipation but not the pain. I have been free from constipation for years but it came back this winter. I can not tell when I am constipated any more because of the gas.
The stool color is good, no blood in the stool. I am neither a smoker or do I drink. I do like one cup of coffee in am and carbonated drink once an awhile. I only drink a high protein drink for breakfast lately and cereal or some carbohydrate later. My BS has been off for awhile, especially in the am. I can't eat much carbohydrates in the early am. ( I have been a border line diabetic for years) My stress level is high which has always affected my BS.. I am a retired nurse and am facing the fact I might have to return to nursing because of our money problems. I also left my friends back in MI, when I married XXXXXXX and we both feel a great deal of hostility here. It about religion. I have been working as a Substitute teacher but there has been little work this year. There is little work in this area but because of our home moving is not that easy. I really like to move on but it not that easy.

Hello Dear,
Sorry for a bit delayed reply.

Constipation and irritable bowel seems to be the main culprit in your case.When food constipates it accumulates in intestines leading to production of large amount of toxic gases(by action of bacteria's and yeast).Linzesss is a stool softner i would suggest you to take laxative for few days.
Laxative irritates the intestinal mucosa and help in excretion of stool including those which is impacted either in rectum or intestine.
Start taking any laxative like lactulose 15 ml once daily before going to bed.
As constipation will settle down gas formation would automatically stop.
Your high anxiety levels are also contributing to your problem.Kindly get a prescription for Benzodiazepene like Etizolam for few days this will help you deal with anxiety.

As of now i would advice you to go for a CT abdomen intially along with stool test to rule out colonic volvolus or Diverticulitis.
Along with that start following the medications i adviced you earlier.
You should start feeling better 5th dose onwards.
