Brief Answer:
surgery is only permanent treatment

Detailed Answer:
Hi XXXXXXX

Welcome to HealthcareMagic.

I have read your query and understand your concerns.

But for an anal fistula surgery is the only cure. Medicines can control the infection and provide temporary relief but it will come back again and again.

If you are not willing for the surgery at present then you can take a course of antibiotics like a combination of ofloxacin and ornidazole for five days and a painkiller to reduce the inflammation.

Also take sitz bath i.e. sitting in a tubful of lukewarm water with 10ml of any antiseptic solution added to it like detol, for 15 mins, three times a day.

This should clear the infection and there are chances that the fistula might get closed, but its not 100% guarantee. Just 10%.

For your motions, you may take any lactulose syp in the night. They are easily available at the pharmacy.

Hope i have been helpful.

Brief Answer:
infection will increase, no doubt

Detailed Answer:
Hi XXXXXXX

If on ofloxacin and ornidazole you still are having abscess formation then your antibiotics need to be changed according to the pus culture and sensitivity.

If you will continue like this then the infection will spread and it may form a complex fistula and/or a high fistula, which are much more difficult to treat even by surgery. So it wont be wise to delay.

There is no time limit by when it may form a complex fistula, it varies from person to person.

Regarding the type of surgery which is best for you, it depends upon the type of fistula you are having and the tract taken by it, for which you need a clinical evaluation and a MRI fistulogram if your surgeon after examining you is of the opinion that it is a complex fistula.

So show to a surgeon for an evaluation and a pus culture test.

Brief Answer:
culture would had been better

Detailed Answer:
Hi XXXXXXX

If you don't have time to go to a surgeon now then just vist a good pathology lab and ask them to take a swab of the pus discharge for culture and sensitivity. The report will come in 2-3 days.

If there is not pus discharge at present then you can start on a third generation cephalosporin like cefuroxime 250 mg or cefpodoxime 200 mg along with metronidazole 400 mg.

Brief Answer:
no

Detailed Answer:
Hi,

Don't take oflox and ornidazole now. Its not working for you.

Metro is to be taken thrice daily and will give a metalic taste in your mouth. Other one twice a day.
