Question : Hi. I'm 51, (5' 8", 127lbs) female and have had burning ulcer pains since mid Dec. Yesterday, I went to med clinic to see P.A. who gave me Zantac and they ran various blood tests. Every morning for few weeks, I wake up feeling totally depleted, like I'm lacking something. This has happened before about 1 1/2 yrs ago and usually lasts a month or 6 weeks, maybe a little longer, but goes away. The only things that make the feeling go away, is eating alot of kidney beans, minestrone soup w/kidney beans, pinto beans, etc, lunchmeat sandwiches w/spinach, etc, and foods that have alot of iron. Lately, it seems this is not quite enough, and I found that drinking water w/sugar in it or juice w/added sugar helps. I was having heavy periods up until 3 or 4 months ago, so I''ve been trying to build up my iron in case I'm anemic. The P.A. took blood to test for anemia, diabetes, low Vitamin D and H. Pylori. It will be awhile before I get results back. Very occasionally, I have had pain across the bottom of my ribs, both side, but worse on my left side. Only lasts a few seconds but sharp, almost piercing, lasted 3 or 4 seconds, (I still have ovaries/uterus, and gave birth to 2 children in my 20's) I have also been going through menopause, and every single morning, around 4:30 or 5:30am, I wake up very hot and my heart pounding really fast, so that I feel it in my chest. Sometimes I feel like I'm falling and it wakes me up. Everytime I close my eyes, I feel like I'm falling, so I usually just get up and it goes away. Horrible feeling. Any idea what causes this? Now tonight, it seems, I noticed undigested beans, in my stool, and stool itself is XXXXXXX and alot of small pieces. Should I be concerned about fast heart beat? And what cause undigested food and XXXXXXX colored stools? It may be a few days before I hear blood test results, but when my heart pounds so fast it scares me. Usually it is only when hotflashes occur, but tonight it is beating faster than normal too. I've heard that fast heartbeat can be caused from anemia, and also occur during menopause for some women and that 400mg of Magnesium is given for this. Would this help relieve heart palpitations? Is there anything I can do?? Thanks! XXXXXXX

Try to avoid any stress in your life. Yoga and meditation will be helpful. Lead a positive life. 6-8 hours sleep will help. You can continue with the antacid you have been prescribed. Along with it you can take Domperidone 30mg for more relief. Anxiolytics like Alprazolam 0.25 can be taken for severe anxiety, under medical supervision only. Following precautions will be helpful :- 1.Avoid spicy and oily foods. 2.Avoid being empty stomach for long periods. 3.Avoid mental and emotional stress. 4.Avoid smoking and alcohol, if any. I would also suggest that you should get following investigations done :- 1. Thyroid profile. 2. 2D Echocardiography. More can be commented after knowing your reports. I hope this is helpful to you. I do yoga stretches and relaxation, take nature walks and believe I lead a fairly positive lifestyle, so I know it's not anxiety. Infact, I was in a wheelchair from MS for over 11 yrs, and my motto has been "I may have MS, but MS will never have me!" I feel famished each morning, like I haven't eaten for days. EVEN after eating bowl of oatmeal, or cereal with fruit, I have to keep eating and eating iron rich foods to make feeling go away. Then, after it seems I have eaten enough nutrients, I'm usually fine for the rest of the day, occasionally it comes back in the evening. Despite everything else going on, Actually I woke up feeling great this morning, which lasted about 30 mins, then that same depleted feeling came back again. It has been 5 hours since I got up and I have been eating and eating different foods, trying to get it to go away. Yesterday it was nearly 4 or 5 pm before I it finally went away and I felt normal again. It seems it's taken longer and longer each day for it to go away. The first time it happened was about 1 1/2 yrs ago, and it always went away with simply drinking some milk. I had to keep sipping on it for 3 or 4 hours. After about 2 or 3 weeks it was completely gone. Then months later it came back again, this time milk was not enough, I would need to eat a can of minestrone soup, which contains kidney beans along milk. Again, after 3 or 4 weeks it went completely away. Months later it came back yet again. This time, milk, minestrone soup were not enough, I had to also consume a bowl of kidney beans, minestrone soup, and milk before it ceased. Same thing again, lasted around a month. This time around, eating all of those things is not enough, but I have added drinking water w/sugar, juice with sugar, soda, or crackers and peanut butter, every 30 mins/ 1 hour, to the list of things. When it first started 1 1/2 yrs ago, it would go away mid morning. Each time it's taken longer & longer to go away during the day. Now it takes 3/4 of the day before it goes away. Eating all of those foods is on top of eating my regular meals, but if I don't keep eating something, I feel faint famished/faint again. The heart palpitations wake me up after about 5 hrs of sleep every morning and I feel it occasionally during the day. It seems I only have the fast heart beat issue during the weeks I'm also experiencing the problem where I have this famished feeling. I was diagnosed about 4 yrs ago with an ulcer, It's flared up a few times since then, after eating things like pizza, pepperoni, etc. Recent Blood Tests came out fine, other than low Vitamin D (was 19). However, when testing for diabetes, aren't I supposed to fast first? I feel like whatever it is, is getting worse as time goes by, because each time whatever this is flares up, it seems it's harder to make it go away and also last for longer and longer periods. Although it has alway went completely away over time, it is really interfering with my day to day life and make me feel awful. Like I said, I was in a wheelchair for over 11 yrs, about a year or so ago, I started walking again, MS is in remission. I was taking walks every day this summer, enjoying life and making up for missed time, sight-seeing, But this has really put a damper on it. Every day I get up in the morning hoping it will finally be gone. This time it's been going on since a week or so before Christmas. Any suggestions will be GREATLY appreciated. Brief Answer: Antacid gel can be added. Detailed Answer: Hi, Thanks for providing more information. I would suggest that you can add an antacid gel about 10 ml after meals. This will help in forming a protective covering over the damaged mucosa of the ulcer area. Also follow the precautions advised earlier. Also get yourself investigated as advised in my previous post. Thankyou much Dr. R.A. XXXXXXX This morning I noticed the left side of the bottom of my ribcage appears very slightly larger than the right. Still not feeling quite right today. I will be going back to see Dr. Just wanted to run this past you to see what you think. What could swelling in this area mean, with the feelings I've been experiencing, could this be related to pancreas? Should I have them check something about the pancreas or what should I do next?
