Question: Dear doctor,
I have been researching about the course of disease left sided heart failure.

For example the course of disease for ischaemic strike-left side is
With referfusion- blood restore to area, many symptoms gradually resolve
Without treatment - ischaemia extends to penumbra- symptoms worsen. Recovery may continue 6 months to a year but left with disability. requires rehabilitation to optimise function.

Thank you very much for your time. If possible can you please elaborate the course of disease for left sided heart failure. A recent link or some sort would help also.

Dear Dr lliri,

Thank you for the reply. As I said earlier I'm doing a research regarding left sided heart failure.

About a 78yrs female, her symptoms were severe dyspnoea tachycardia, Spo2 85%, hypertension, tachypnea, bilateral basal crackles and her ECG showed atrial fibrillation. Thank you very much for your time.

It seems that we have to do with a decompensated chronic heart failure presenting with signs of congestion.

Coming to this point, it is necessary to properly and timely control the potential triggering factors (uncontrolled hypertension, afib with rapid uncontrolled ventricular response, an acute infection most commonly pulmonary infection, acute anemia, new episode of cardiac ischemia, etc.)

Sometimes, even a bad compliance with therapy may be the reason of clinical status deterioration.

That's why it is necessary a comprehensive clinical investigation considering the following tests:

- complete blood count,
- thyroid function tests,
- fasting blood glucose,
- resting ECG,
- cardiac enzymes (CK-MB, Troponin),
- BNP, NT-proBNP,

- blood electrolytes level,
- cardiac ultrasound,
- chest X ray study,
- PCR & ESR,
- total plasma protein
