Suggest Treatment For Chronic Headaches

Question : Hi,
I've been having head aches for 3 months and went for an MRI of my brain in Israel and confirmed in Canada and it was found that the changes on the MRI represent chronic ischemic changes and are not suggestive of multiple sclerosis or vasculitis.it seems to be related to my age and high blood pressure for a year not treated with meds.
On July 13 -bp:(170-155)/(94/84)
I started taking conversyl 4mg 1x a day and monitor my BP and had to change it to 8 mg reccomended by my cardiologist On July 14 I changed it to 8 mg and only on July 17 it started going down(144-134)/(/90-83)

Yesterday morning as in the last5 days the BP has been low :(133-116)/(84-72)and my headache less strong but still there.The neuro who saw me in Israel it is tension headache .

I've been feeling some side effects from the conversyl 8 mg and saw my gp since my cardiologist is on vacation. I felt pressure in the chest, caughing, feels like the throat is swallen...
He changed my med to:

Olmetec 40 mg

I took it last night for the first time

This morning:
My blood pressure went back to high:

153-143/92-85

I'm very worried: cannot have bp high because of my problem in my brain and my gp is not working till Monday.

Questions:


1)Should I go back to conversyl 8 mg till I see him on Monday?

2) can the BP change so fast?
3) if going back to the conversyl can it take 5 days again to lower it?
4) if staying on olmetec can it take 5 days to lower even if it was lower because of the conversyl?

I passed carefully through your question and would explain as follows:

Your symptoms (swollen throat, chest tightness, cough) seem to be related to angioedema, which is a known adverse effect of Olmetec too, but it is more rare compared to Conversyl.

As Olmetec needs a prolonged time (2-3 days) to show its action, it is common to have these blood pressure fluctuations, as you have stopped abruptly Conversyl. For this reason, I would recommend taking the next dose of Conversyl today, although you have started Olmetec, which has not showed it effect yet. You can continue until Monday with Conversyl and Olmetec and then stop Conversyl.

Conversyl does not take so long to show its effects, because you have missed only one dose. So, there is no reason to worry about!

As you did not tolerate Conversyl, I would not recommend continue it in the future (except for these days), because there is a wide variety of anti-hypertensive drugs, which you can take without showing this adverse effect.
